Vote By Mail

An executive order issued by Governor Andrew Cuomo is now allowing all New Yorkers to vote by mail in our primary on June 23rd. The Board of Elections will mail an application or you can download it here.
Requesting an absentee ballot is easy. The steps to request your ballot, as outlined by the Suffolk County Board of Elections, are:
  1. Download an absentee ballot request form (English FormSpanish Form) or use the one that arrived in the mail

  2. Fill in temporary illness box in section 1

  3. Provide all other information on the form

  4. Save and email the form as an attachment to
    or print and mail to Suffolk Board of Elections, PO Box 700, Yaphank NY, 11980
