Vote By Mail

Requesting an absentee ballot is easy. The steps to request your ballot, as outlined by the Suffolk County Board of Elections, are:

  1. Download an absentee ballot request form (English Form, Spanish Form) or use the one that arrived in the mail
     

  2. Fill in temporary illness box in section 1
     

  3. Provide all other information on the form
     

  4. Save and email the form as an attachment to boeinfo@suffolkcountyny.gov
    or print and mail to Suffolk Board of Elections, PO Box 700, Yaphank NY, 11980
